This new Romanian book on modern surgical difficulties has a terrific chapter on medical futility: "Therapeutic Proportionality and the Limits of Surgical Intervention."
It repeatedly emphasizes the subjective nature of "benefit" that is enough "to justify the aggression associated with treatment." As surgery has become more technically advanced, "the gap between procedural success and the real value of the outcome has become increasingly apparent."
