Here is a musical patient safety reminder:

• An advance directive does not automatically mean the patient is DNR. Read the advance directive. 
• An POLST does not automatically mean the patient is DNR. Read the POLST.
• DNR does not mean do not treat. It applies only to CPR, not other therapies.
