Hospitals and community clinics in Colorado must post "Service Availability Forms" that identify any health-care service that they refuse to provide for "non-medical reasons." Around 150 are now on the CDPHE website.
The form focuses on reproductive health-care services, LGBTQ health-care services, and end-of-life health-care. Notably, the end-of-life health-care section includes sections on not only MAID and life-sustaining treatment but also on VSED.
